Rhinoplasty?

The face is a kind of human calling card, and the nose is its central part, so a harmonious, beautiful facial expression highly depends on its shape. Changing the shape and size of the nose can not only improve facial proportions but also improve breathing. Rhinoplasty (also known as rhinoplasty) allows for changes in nose size, profile, tip shape, nasolabial angle, narrowing of nostrils, and restoration of a nose damaged by trauma, as well as straightening of the bony and cartilaginous septum. If only minimal changes are needed, non-surgical nose correction is often performed. In this way, nasal imperfections are corrected by injecting tissue fillers and using surgical suture procedures. It is precisely in this way that the tip of the nose is corrected, curvature is straightened, a hump is reduced, and the tip of the nose is lifted.

Reasons for seeking nose correction

Most often for the correction of the nose shape itself, which can be congenital (e.g., a hump or an unbalanced nasal tip). People also often seek this procedure when the nasal septum and nasal axis are deviated due to trauma or other reasons.

Nose plastic surgery is performed on people whose facial growth process has been completed. It is most often performed when the patient complains of a large nasal hump or, conversely, has a „flat“ nose. Nose reduction surgery is often performed when the patient complains of an overly long nose. Nose correction can also be performed when it is crooked, potato-shaped, disproportionately too short, too concave, or too bent.

Do more men or women seek rhinoplasty?

Women seek it more often, but this surgery is also frequently performed on men. Usually, men seek it for crooked nose correction, especially after previous injuries, while women seek it for aesthetic reasons.

Before a rhinoplasty surgery, it's important to discuss the following questions with your doctor:?

First, it would be good for the patient to clearly articulate their desires – how they envision a beautiful nose, perhaps have examples, and explain why that particular shape is beautiful to them. The surgeon, for their part, must answer whether it is possible to achieve this, show the results of their work or expected outcomes. Expectations need to align with possibilities. One must discuss with the surgeon how long it will take to see the result, and what could happen that would prevent the expected outcome from being achieved. To better understand the expected result, 3D modeling can be performed.

The reasons can be diverse, such as general illnesses, especially those related to bleeding problems, age can sometimes be a limiting factor, sometimes it can be difficult to achieve a good result when the skin and subcutaneous tissue are very developed, oily, and thick – in such cases, surgery cannot always be performed, as the desired result may not be achieved due to the scarring process, and the nose remains swollen and puffy for a long time.

Is it possible to predict the outcome of a future nose surgery?

It's only possible to estimate – special programs are designed for this, allowing simulation of what to expect. Usually, a surgeon can say they will definitely make the nose shorter, more upturned, and without a hump, but whether it will match that imagined beautiful nose is a big question. By analyzing various beautiful noses, especially those of women, you'll notice that they all share basic elements of a beautiful and attractive nose.

Is nighttime plastic surgery a safe operation?

Every operation carries a certain risk. A nose operation is not very complex or life-threatening, but from a surgical perspective, it is technically difficult due to the fact that it is often hard to achieve the desired result, requiring the surgeon's experience and luck to achieve an ideal shape. For the patient, it is related to whether the obtained result will meet their expectations.

Is there any possible side effect of the operation?

Nose surgery, especially septum correction, is associated with improved breathing. A nose alteration significantly enhances respiratory function, allowing individuals to breathe fully, which greatly improves overall well-being. After a septoplasty, the passage through the previously damaged nostril „opens up,“ and this improvement is very noticeable.

With which operations or procedures can rhinoplasty be combined?

Nose surgery can be combined with nasal septum correction, double chin correction, facial injections, and body procedures such as breast implants. Rhinoplasty cannot be performed in conjunction with eyelid surgery or a lip lift.

How is a nose job performed??

Nasal surgery is usually performed under general anesthesia, although sometimes sedation is used when only the tip of the nose is being corrected. The surgery can be performed using an open or closed rhinoplasty technique. In open rhinoplasty, a 3 mm incision is made in the columella between the nostrils, and incisions are made inside the nostrils. In closed rhinoplasty, incisions are made only within the nasal mucosa. During the surgery, the bony and cartilaginous parts of the nose are corrected, and various surgical maneuvers are performed to restore the nasal axis, remove a hump, and reshape the nasal tip.

We have now started using an ultrasonic piezo scalpel in the clinic, which is designed for nasal bone correction. Thanks to it, it is possible to avoid significant bruising and swelling that usually accompany patients after surgery, and a much smoother, narrower bony part of the nose (bridge) is achieved.

How long does a nose job take?

Nose surgery lasts from 1 to 3 hours. Surgery on the tip of the nose alone can take 1–1.5 hours, while a complete rhinoplasty can take 2–3 hours.

Is the nasal surgery painful?

What method of anesthesia is used during a nose operation??

Nose surgeries are performed under general anesthesia (with mechanical ventilation); only when performing a tip rhinoplasty is it sometimes possible to limit it to sedation.

Possible complications after rhinoplasty

Complications can be general, common to all surgeries, or specific to rhinoplasty itself. In the case of nose surgery, the greatest concern in the first few days or immediately after the operation is bleeding, as the nose is a well-vascularized organ and bleeding would be an unpleasant problem. There is always a risk of infection, but it is quite rare in the nose, as the face is well-adapted to fighting infections. In later periods, it is not so much complications that are concerning, but rather the patient's disappointment with the long healing process, which can take a very long time, so patience is required while waiting for the final result.

Post-nasal surgery care

After surgery, the patient spends one night at the clinic. The so-called tampons (long cotton plugs) are removed from the nose the next day, stitches are removed after five days, and a special brace is removed after 10–14 days when the bones have healed. The patient can return to work after 2–3 weeks. The initial result of the nose's shape is visible 4–6 weeks after surgery, but the final result, especially in the cartilaginous part of the nose, takes up to one year, sometimes even up to two years, to form.

Sick leave after surgery

If a full rhinoplasty is performed with bone grafting, a special splint will need to be worn for at least 10 days (sometimes up to 2 weeks). During this time, the person will not be able to go to work, even if they feel well after a week. If only the tip of the nose is operated on, it will be possible to return to work after a week, if there are no bruises.

How long does the healing process take??

If only the tip of the nose is operated on, the wound heals in 5-7 days, and a preliminary result can already be seen. If the bony part is also operated on, a splint will be worn for 10-14 days. After its removal, we will see the preliminary result. However, this is not the final outcome - it is a preliminary, initial result.

When can the final result be expected??

The nose healing process is related to post-operative scarring. Sometimes it can take 6-8 weeks, and sometimes you may have to wait six months or even up to 2 years. Usually, six months after a nose surgery, with smooth healing, we already have an almost normal appearance. Recently, a new methodology has been applied, and the almost final result without significant swelling is sometimes visible even after 6-8 weeks.

Will there be scars after the surgery?

Yes, scars will remain, but you don't need to worry about them, they are not noticeable. If operated on openly, a scar will remain on the nose Columella or in the middle section. If the nostrils were reduced, the scar may be at the base of the nostrils. These scars are not very noticeable, as there is a natural fold at the nostrils, and at Columella This place is covered in shadow and is also almost invisible.

Revision rhinoplasty may be necessary in cases such as:?

In case of early bleeding, revision may be required to stop it. Later, repeat surgeries are associated with additional corrections, unsatisfactory scarring, and an undesirable final appearance. Then, additional corrections may be necessary, but they can only be performed no earlier than one year after the surgery.

Nose surgery is one of the most complex aesthetic procedures, not only because the nose is a central facial feature that determines the entire appearance, but also because it requires extensive surgical experience, an understanding of aesthetics, and luck. No other area of the body is as dependent on the postoperative scarring process, which determines the final result, as the nose. For this reason, the surgeon must, based on their experience, predict the final result, which may only form after 2 years. The cartilaginous part of the nose is particularly capricious in this regard.

Despite all these circumstances, rhinoplasty is a particularly effective aesthetic surgery, fundamentally changing facial appearance, and sometimes a person's entire life.
There are two approaches to rhinoplasty: open and closed. Both have their own pros and cons. With the closed approach, the internal nasal structures are not clearly visible, making this method more suitable for minor corrections in the bridge area. With the open approach, all nasal cartilages are clearly visible, allowing for a wide range of corrections, which is especially important for the tip of the nose. The final result of surgery performed with the closed approach forms faster than after open rhinoplasty.

Actions during rhinoplasty

During rhinoplasty, when correcting the cartilage area, part of the cartilage is removed; the cartilaginous dorsum and the tip of the nose are shaped. The bony part is corrected by shaving off the bony hump, performing lateral osteotomies, and bringing the bones together. In this way, a crooked nose after trauma can be straightened, and a narrower nose without a hump can be formed.

After rhinoplasty, the nose tip covers the newly formed structures. Although there is often a slight excess, it shrinks over time and „tightens” around the new framework. Once the post-operative nasal shape has formed, various minor irregularities may become apparent, which can now be very easily corrected with filler injections.

It's important to note that fillers can also be used for slight correction in the tip of the nose area – it can be lifted, the nasolabial angle can be adjusted, and the bridge can be straightened.

So, rhinoplasty is a very effective cosmetic surgery, the result of which highly depends on the surgeon's accumulated experience, the patient's and doctor's understanding of aesthetics, and the postoperative scarring process.

Recently, I've started using special additional surgical techniques that significantly reduce scarring, resulting in a noticeable improvement within 4–6 weeks. This is very pleasing for both patients and myself. New methods of wound packing have also been implemented, using absorbable dressings that greatly facilitate the uncomfortable first dressing change after surgery.

After the operation, we want to see the patient until the wound has fully healed – preliminarily, this is 5 days. After this period, the wound is redressed, the sutures are removed, and although a splint may be left in place, the patient can already fly on an airplane.
